- Solo travelers need a 6-month personal bank statement (minimum balance PKR 1,000,000) on the bank's letterhead, including bank details, and for families (minimum PKR 2,500,000). Avoid sudden deposits.
- Personal Income Tax Returns / Tax deduction certificate of last 3 years
- Proof of Proprietorship / Partnership (In case the applicant is a Proprietor / Partner in a firm)
- Company Registration Certificate / Certificate of Incorporation
- Chamber of Commerce Certificate if you have a membership with the Chamber of Commerce
- Company Income Tax returns for the last 3 years (If filed separately)
- Original Business Current Bank Account Statement for last 6 months mentioning the Business Name and Account No. on the Bank's letterhead with the seal and sign of the bank officer.

- Please deposit the latest bank statement, which should not be more than 7 days old at the time of submission
- For minor applicants, visa form & other supporting forms should be signed by both parents along with a Signature or Thumb impression of the Minor applicant as appearing on his/her passport
- The amount you are paying for the file assistance charges excludes 5 years multiple entry visa fees for the United States of America
- The fee for US Embassy is currently 185 USD (equivalent to PKR 55,500)
- Appointment processing time for US is 6 week
- Appointment Expected Availability within 120 days
